After submitting this form you will get a e-mail with in 72 business hours in regards to setting up your in person consultation as well as a Price estimate for your particualr hair needs. The In-Person consulation is so necessary to ensure we match your current hair to the hair of your dreams. I understand that by submitting this form it does not guarantee an appointemnt. I understand by submitting this form that I will recieve general pricing and information via email with-in 72 business hours of the date this is submitted based on the  answers I provided. I underdstand that in order to be booked I must also schedule a In-Person consulation with Molly at Relax Nail bar and salon along with this Extension Consulation Application Form. I understand that it may take up to 2 weeks to recieve a consulatation appointment and that price can very greatly depending on the length, fullness and extension method used. I understand the extensions are considered a luxury service and therefore are a Time Commitment as well as  Financial Investement.I understand that install/reinstall appointmetns must be kept every 6-8 weeks. I also understand that a 50% Non Refundable rentainer fee will be required before my initial install appointment can be scheduled.I also understand once the initiall install appointment has been scheduled, should I choose to cancel my appointment I Forfiet the rights to my retainer fee.
