I hereby attest I am renting a GPS Tracker from Professional Investigations. I agree to take all responsibility for this device. I agree that if the tracker is lost, stolen or damaged in any way, including the tracker falling off the vehicle, I will reimburse Professional Investigations the amount of $450. If Professional Investigations conducts GPS surveillance only, I will be charged a rate of $45.00 per hour for that service. 

I understand there is a GPS activation fee of $65.00, per unit, which is charged at the beginning of your case and subsequently the first of each month, thereafter, while in possession of the GPS device. If an investigator is on surveillance tracking the GPS, data usage may be required while away from a Wi-Fi location. In this instance the client will be responsible for data use costs. While in possession of the GPS but not in use to conduct surveillance, there is a fee of $15.00 per day. I understand there is a one-time fee for a successful deployment and also retrieval of the GPS at the start and end of my case of $105.00 plus $.70 per mile each time of travel. If an attempt is made and is unsuccessful, I will be charged an hourly fee of $60.00 per hour plus $.70 per mile for each attempt. If during my case the GPS indicates a low battery, I will be charged a fee of $105.00 per battery change plus $.70 per mile. During normal use the battery should last 30 days. 

By signing this agreement, I attest that there are no 50B’s or other active protection orders associated with this case.
