Incomplete forms will not receive a response. In order to give a proper response, I need all of the information in the form completed.
Booking-
All appointments require a $200 deposit to hold the spot. Multiple day appointments require a deposit for each day. This will come off of the final cost of your tattoo. If you need to reschedule I require a 72 hour notice or your deposit will be lost. Deposits are non refundable.

Where on the body are you wanting the tattoo placed?
Please attach a photo of where you would like to place the tattoo. This photo will be used as a template for designing your tattoo. It is important to take the photo from a square on position. It will also help determine design aspects based on skin tones. The more accurate these two pieces of information are, the better the design.

-Make sure you eat before you come to your appointment and bring snacks as well. Also it will help you to hydrate and rest the day before.

-Feel free to bring headphones, a book...anything to help you pass the time and keep your mind preoccupied.
