Signature - USER GROUP GUIDELINES AGREEMENTPLEASE NOTE: THIS APPLICATION SUPERSEDES ANY PREVIOUS VERBAL OR WRITTEN AGREEMENT OR COMMUNICATIONGUARANTEE AND FINANCES: Groups desiring use of the facilities at Camp Alamisco must complete this application form. Confirmation of your event will be sent upon receipt of the deposit and Rental Application. The deposit is required to complete the reservation process. The amount of deposit is 10% of the estimated reservation fee. Please return the required deposit along with your completed application within 15 days of receiving this Rental Agreement Packet. If deposit is not received within the stated deadline, Camp Alamisco reserves the right to cancel the confirmed dates. CANCELLATION POLICY: The rental agreement may be cancelled at any time up to 60 days prior tothe reservation date with 50% return of deposit. Cancellations made 30 days prior to the reservation date will result in forfeiture of deposit. If cancellations occur within 5 days prior to the reservation date, the group will forfeit their deposit and be responsible for 100% of the estimated cost of foodbased on the expected expenses. THE BALANCE OF RESERVATION FEES IS DUE UPON RECEIPT OF INVOICE. We, the undersigned, have filled out the Rental Application to the best of our knowledge. We have read the above User Group Guidelines Agreement, Requirements For Off-Season Guests, policies of Camp Alamisco, and the regulations concerning finances. We agree to these conditions and request use of Camp Alamisco for our event.
