Congratulations on making the first step towards joining our Competition Dance, Competition Cheer and Showtime Performance Teams in 2021 within our Enhancement Program. We're thrilled you've chosen to partake in this opportunity and look forward to creating many wonderful memories. This Commitment Form is an agreement that the listed student is committed to your selected Enhancement Program Teams in 2021 and that students and families acknowledge the commitment and financials required and that upon attendance at our Placement Day, you will be placed in a Competition or Showtime Team.

MJDS Enhancement Program - Team Commitments and Information
I acknowledge that the Competition and Showtime Package is 11 equal monthly installments, payable in February - December inclusive and I must be kept financially paid up-to-date.
I acknowledge that the Competition and Showtime Teams have additional classes that must be met, and can be found in the Enhancement Program Information
I acknowledge that the Competition Teams have 2-4 competitions and the Showtime Teams approx 3-5 Performances during the 2021 season set out at the beginning of the year for each team in which I must attend all of them, however this may also be changed pending COVID-19 regulations and availabilities
I acknowledge that once I attend the Placement Day I will automatically be placed in a Competition and/or Showtime Team. Should I wish not to accept my given team, written notification must be given to maxijazzdancestudio@hotmail.com by Sunday 10th January of my withdrawal from the team
I acknowledge that I have read through and agree to the Maxi-Jazz Dance Studio full Terms & Conditions as provided on the website and the online '2021 MJDS Program Packages + T&C' document

Enhancement Program Placement Day - All students aged 7 and up!. This booking is valid for one placement day for the listed student in the commitment form only. Should you choose to place in both a Dance/Showtime and Cheer Team, you will be required to purchase 2 tickets. All students attending placement day MUST purchase a Placement Day ticket.
