• Course Audit Approval Form

  • Once you have completed the form:

    1. Click the submit button.
    2. Print and have it signed by the Course Instructor.
    3. Return the signed form to the Registrar's Office, Duffy 112 to complete the audit registration process.
    4. Note: Employees wishing to audit a course must also have the form sign by Human Resources in addition to the faculty member teaching the course.
