Photograph and Video Consent Form
The purpose of this parental/guardian consent form is to obtain permission for your child to appear in any videotapes or photographs that are created for the purpose of promoting and improving art instruction or documenting important events during our Community-based Mural Art Project. These will not be shared on social media or in any public forum. Any documentation will only be shared during virtual sessions, my class, and professors.
