Please upload (at the end of this form) a declaration from the egg processor, that all eggs used in the egg product processing, come from farms, that on the date of processing, are clear of Avian Influenza and have been clear for 30 days prior to processing. Also that there have been no cases of Avian Influenza within a 30 mile radius.
