What is Face Feet and Fanny (FFF)?
FFF is a light trim of the face, feet, and fanny areas of your pet to remove excessive or tangled fur. A bath is accompanied by the FFF added charge. Nail trim and ear clean are included.

TIPS ARE GREATLY APPRECIATED. Services are performed by a trained professional INDEPENDENT GROOMER.

Vaccination: All animals must be up-to-date on required vaccinations

Dogs: Rabies, Bordetella, and only if never been given/boosted then the DA2P.

Cats: Rabies, FVRCP.

Parasites: Pets must be free of external parasites (ticks, fleas, etc.) or they will be treated at owner's expense if found on the pet.

RULES AND POLICIES FOR GROOMING

1. New pet patients are required to have a PRE-GROOMING EXAM to establish as patient if not already an exising patient.

2. All pets admitted must be current on their vaccinations:

FELINE: RABIES & FVRCP;
CANINE: RABIES & BORDETELLA

3. Any pet found to have FLEAS or TICKS will be treated at the owner's expense.

4. At Parkland Veterinary Hospital, there is nothing that is more important to us than the health and well‐being of the pets in our care. Your pet is important to us. Because we care about your pet’s safety and well-being, we want to assure you that every effort will be made to make your pet’s visit as pleasant as possible. Occasionally, grooming can expose a hidden medical problem or aggravate a current one. This can occur during or after grooming. If your pet is severely tangled or matted, it is at greater risk of injury, stress and trauma. All precautions will be taken. However, problems occasionally arise, such as cuts, nicks, clipper irritation and mental or physical stress. In the best interest of your pet, we request your permission to obtain immediate veterinary treatment should it become necessary. I release the Groomer, Parkland Veterinary Hospital P.A., and its staff from any and all claims, damages, liability, accident, illness and injury.

5. I further acknowledge that I am completely responsible for the actions of myself and my pet(s).

6. This agreement applies to all current and future pets that will undergo grooming services at Parkland Veterinary Hospital.

Sedation Consent: Review Only If Requesting Sedation
SEDATION and/or TREATMENT CONSENT1. I do hereby certify that I am the owner/guardian/agent of the pet described above and have full authority to execute this consent. I give my full authorization and consent to have the veterinarians of Parkland Veterinary Hospital to perform the sedation or treatment as requested or discussed.2. I authorize the doctors and supporting staff of Parkland Veterinary Hospital to perform services, diagnostic procedures, and treatments deemed as necessary to improve my pet’s quality of life and provide quality veterinary care. I authorize the use of all anesthetic agents, sedatives, tranquilizers, and other medications and supportive care before, during, and after my pet’s procedure deemed as necessary by my pet’s attending veterinarian. I understand that hospital support personnel will be employed as deemed necessary by the attending veterinarian.3. I have been advised of the risks and possible complications of my pet’s procedure and that results and/or expected outcome cannot be guaranteed. If for any reason an emergency situation arises with my pet, the doctors and supporting staff of Parkland Veterinary Hospital have my full permission to provide medically necessary treatment, and I agree to assume full financial liability for any and all expenses incurred. I understand that payment is due at the time of service and that I am fully financially responsible for any and all services rendered. I also agree to pay for all expenses incurred to collect the debt including, but not limited to, attorney fees, collection agency fees, and billing fees.

End of Treatment Sedation Consent
I hereby grant permission to Parkland Veterinary hospital to obtain emergency veterinary treatment for my pet at my expense if the need arises. I have read, understand, and accept the above grooming policies. Also, I have previously been informed of all grooming rules and regulations:
