  • Your pet needs a current vaccination, flea treatment and worming for their protection, and others in the house. Please let me know if youwish for me to supply/administer either of these.

  • By signing this form, you acknowledge that you are the legal guardian or owner, and the information you have given is accurate and true to your knowledge. In the case of emergency or sudden illness, you consent to treatment given as necessary by your veterinarian stated above, or a more appropriate located veterinarian if needed.

    You consent to veterinary costs incurred unless agreed otherwise. In the event of emergency or illness, Veterinary Nurse Services agrees to contact you as soon as possible at the above phone / email. In your absence you give permission for Vet Nurse Services to seek necessary help and treatment.

    In return, we agree to provide your pet with the best care we are able to, using vet nurse experience experience to make the right choices for their welfare. We aim to provide a fun, and stimulating environment that will promote mental, emotional and physical health.

  • TERMS & CONDITIONS

    The owner is responsible for providing items required for care of their pet, including (but not limited to): bowls, collar, lead, food, treats, bedding, toys, poo bags, litter tray & litter etc.

    A half-day will be charged if the homestay starts after 4pm or concludes before 11am. Otherwise, a full day will be charged.

    The utmost care will be given in watching both your pet and your home, but due to the unpredictability of animals, we cannot accept responsibility or liability for any damage to the pet other than in the circumstance of wilful neglect. Examples include (but are not limited to): bites, injury, disappearance, illness, death.

    You understand Vet Nurse Services need to obtain your pet’s previous medical history.

    Vet Nurse Services are able to provide medical/emergency treatment for the pet, in which case they are able to approve such treatment at the advice of another veterinarian. Attempts will be made to contact the owner, but if the owner is unable to be reached, Vet Nurse Services are able to make decisions with regards to care. The client is responsible for the costs.

    Should any additional costs be incurred, the client will be invoiced at the conclusion of the sit and payment will be made within 2 weeks. Examples include (but are not limited to): mileage for appointments, purchase of additional items for pet care, purchase of items required for care of the home etc.

    Vet Nurse Services reserves the right to terminate this contract at any time before or during its term. If Vet Nurse Services determine the pet poses a health and/or safety risk and the concerns prohibit the carer from caring for the pet, the client will be contacted and responsible for making alternative arrangements. If non-contactable, the pet will be placed in kennel/cattery.

    The client agrees to notify Vet Nurse Services of any concerns within 48 hours of the homestay end date. Any concerns beyond this time will not be considered.
