Thank you for your interest in joining the team at the United Jewish Council of the East Side (UJCES)! Please complete this application form to proceed with potential group volunteer opportunities with UJCES.

The information on this form will be kept confidential and will help us find the most satisfying and appropriate volunteer opportunity for your group.

Please list your group’s preferences for dates/times to volunteer with UJCES. Please also note that our volunteer opportunities are offered on weekdays. UJCES will do our best to accommodate groups based on our agency’s schedule and needs.

Section 3/3: Participant Information

Minimum age requirement to volunteer is 14 years old. Groups of volunteers ages 14-17 may serve with a parent or legal guardian present, and with a signed Parent/Guardian Consent Form for each participant.

Do any members of your group have fluency in Spanish, Cantonese, Mandarin or other non-English languages? (Not a requirement)

Any mobility accommodations or physical limitations for group members? (Some tasks include lifting heavy materials)
If your group is confirmed and scheduled to serve with UJCES, all volunteers must also complete, sign, and return the UJCES Volunteer Agreement and Release of Liability form before volunteering. This will be provided by UJCES before your volunteer activity.
