    • Informed Consent and Acknowledgement

      I hereby give my approval for my child’s participation in any and all activities prepared by Second Chance Ministries/Camp Penuel during the selected camps. In exchange for the acceptance of said child’s candidacy by Second Chance Ministries. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Second Chance Ministries/Camp Penuel and all its respective volunteers, agents, and representatives from any and all liability for injuries to the said child arising out of traveling to, participating in, or returning from selected camp sessions.

      In case of injury to said child, I hereby waive all claims against Second Chance Ministries/Camp Penuel, including all counselors and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all outdoor activities, including basketball, volleyball, gaga ball, 9 square, etc. Some of these injuries include but are not limited to, the risk of fractures, paralysis, or death.

    • Medical Release and Authorization

      As Parent and/or Guardian of the named camper, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

      Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named camper. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

      Permission is also granted to the Second Chance Ministries/Camp Penuel and its affiliates including Directors, Counselors, and Leaders to provide the needed emergency treatment prior to the child’s admission to the medical facility.

      Release authorized on the dates and/or duration of the registered season.

      This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

      BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.

      Code of Conduct

      1. The behaviour exhibited at camp should always be kept within Christian principles.
      2. Respect for church services, each other, and yourself is a must.
        The use of alcohol, illegal drugs, tobacco or marijuana products of any kind including electronic cigarettes are absolutely prohibited.
      3. Absolutely No pranks that hurt feelings, people,  property of others, or campground. Be mindful of others.
      4. NO PUBLIC DISPLAY OF AFFECTIONS! MAKE ROOM FOR JESUS!
      5. All campers must sleep in own beds, bunking together is not permitted.

      Boundary Limitations

      1. No camper or worker will be permitted to leave the campground without permission from the Camp Director.
      2. Absolutely NO swimming or wading permitted in the lake at any time, unless given permission by counselor or Camp Director. 
      3. No camper or worker is permitted in the dorms of the opposite sex.
      4. Sr. campers are not permitted in or around Jr. camp dorms unless directed by a counselor.

      Dress Code

      1. Clothing at the camp must be modest Christian attire.
      2. Shorts need to be mid-thigh or longer in length. Skirts need to be knee-length or longer in length.
      3. Shirts must completely cover the stomach and chest.
      4. Clothing must completely cover all undergarments. 
      5. One-piece or modest tankini swimwear are permitted with the approval of their church leader.
      6. Boys are required to wear shirts or tanks at all times. Nothing that shows the chest or belly. (cut-offs)
      7. Shoes must be worn at all times except while swimming.
      8. No inappropriate graphics or wording on clothing.

      ALL ATTIRE IS SUBJECT TO THE DISCRETION OF THE COUNSELOR ACCORDING TO THIS DRESS CODE. CAMP DIRECTOR HAS FINAL SAY.

      Electronics and Devices


      We are NOT responsible for lost or stolen property.

      1. No electronic entertainment devices should be brought to camp.
      2. Cell phones must be turned in at registration.

      Workers

      1. All camp workers must have given their life to Christ and show a Christian lifestyle.
      2. All workers are subject to background checks.

      Corrective Guidance


      If these expectations are not met, corrective guidance up to and including dismissal from the camp without refund may be necessary. 

      Counselors have discretion over improper conduct.

      Camp Director has the final say.


      Visitors

      1. All visitors must check-in at registration before entering the campground.
      2. Visitor tags must be worn and visible at all times.
      3. All visitors are required to leave the grounds no later than 30 minutes after evening service.
      4. NO visitors, after registration, are permitted in the camp dorms or swimming area at all.
      5. All visitors are subject to the expectations listed above.

       

       

      By signing, I acknowledge that I have read the above expectations and agree that my child will abide by them.

    •     I hereby give consent to Second Chance Ministries/Camp Penuel (the “organization”) to photograph, videotape, or otherwise digitally record and use images and/or sound recordings of myself or my child or children (if applicable) to use in any public media, including radio, television, internet, social media, print, or in any of the organization’s or its partners’ publications, productions, or posts.

          I understand that the intended use of such images and information is solely for the purpose of advertising, marketing, fundraising, and/or promotional and public awareness purposes for the organization. I hereby waive any rights or interest in the images or recordings, as contemplated in this release.

          I acknowledge that this consent to use images and/or recordings is being made solely for the benefit of the organization and comes without any expectation of monetary compensation or other benefits to me. To the extent that any benefit accrues or might accrue to the organization from the use of images or information, I hereby and forever waive any interest in or claim to such benefits.


          I hereby release and forever discharge the organization (including without limitation all corporate affiliates and officers, directors, trustees, donors, employees, agents, and volunteers) from any and all claims, liability, actions, suits, demands, costs, expenses, or indebtedness arising out of, related to, or in any way connected with the use of images and materials described herein, and I hereby waive all rights and interests in and to such information and materials.


          I further acknowledge that there is no guarantee that any or all of the participants’ images or recordings will be used in any released media.
