HSC Year 5 Afternoon Tea Catch Up
Dear Parents and Guardians,

Date: Thursday 19 November

Time: 2.15pm-3pm

Venue: Resource Centre Verandah

For Parents to attend the HSC Year 5 Afternoon Tea Catch Up, Concordia is required to strictly follow the Queensland Government COVID-19 guidelines for school events.

All guests must be pre-registered in household groups using this form. (People who live in the same household are considered to be a household group.) All guests are required to practice social distancing of 1.5 metres with other household groups at the event.

All guests must be registered using this form before the 18 November so that we can organise the seating on the verandah. Guests are NOT able to register after this date.

All guests must enter via the gate near the flag pole and sign in on the attendance register.
