Please review the list of documents you will need to complete your application. You cannot save your application and return to it later. So be sure to have your documentation when you are ready to complete and submit your application.
Provide documentation so we can verify your income. Documents include:
4 most recent pay stubs for all employed household members,
2019 tax return, AND
most recent checking and savings bank statement(s)
recent eligibility letter/benefit statement for SNAP/WIC, Section 8, Unemployment, Child Support, Social Security, etc.
Note: You must submit income documentation with your application even if it is already on file with Fuel Assistance, Head Start, or other SSCAC program. Please be sure to cross out social security numbers and account numbers on all documents.
If you are applying for rent assistance, you will need to upload a copy of your current lease and provide your landlord's contact information.
If you are applying for mortgage assistance, you will need to upload a copy of your most recent mortgage statement with payment voucher and your lenders contact information.
If you are applying for another type of basic household assistance, you will need to provide a copy of your most recent bill, invoice, or quote.
