• As a client/consumer of Phoenix Preferred Care, Inc. you are responsible to:

    1. Actively participate in the development of your treatment plan.

    2. To follow through with your treatment plan recommendations.

    3. To inform your service provider of any problems you have in following your treatment plan.

    4. To inform your service provider of any problems you are experiencing with medications that have been prescribed to you.

    5. To inform your service provider of any changes related to medical coverage (i.e. medical card denial, insurance changes) prior to your next scheduled appointment

    6. To keep and participate in your scheduled appointments. Contact Phoenix Preferred Care at 451-9379 at least one day in advance if you

    need to reschedule. In the event that there are 3 missed

    appointments, you will be responsible to contact the office at (606)451-9379 to schedule additional appointments.

  • 9. Adhere to the Financial Policy listed below: Kentucky Medicaid Clients: We will file Medicaid claims for your outpatient services. In the event that your assigned Managed Care Organization requires a co-pay, you will be responsible for this prior to your scheduled appointment. Your Managed Care Organization requires that Medicaid eligibility be active on the date of service. Ineligibility will result in the cancellation of future therapy appointments unless deemed an emergency. Questions regarding this policy can be directed to the site supervisor.

    Private Insurance Clients: Phoenix Preferred Care, Inc. participates with several private insurance carriers. Verification of Co-Payment or Deductible will be verified upon intake. Any required payment for services (per your policy) is expected to be made on the date of service, or you will be asked to reschedule. Questions related to private insurance may be addressed by contacting PPC billing office at 451-9379.

    Private Pay Clients: Our practice is committed to providing the best possible treatment for our clients. The amount charged to the client is usual and customary for our area. Phoenix Preferred Care offers a sliding fee discount for those who qualify. You are responsible for payment on the date
