Pet Service Agreement

I   First Name   Last Name  agree to the terms outlined by the Company to take care of my pet   Pet Name   between the days of   Date   and   Date . 

I agree to pay all of the calculated cost before our services start , totaling   Dollar Amount . 

Please individually initial the following statements: 

• I agree to adhere to the Company’s 72-hour cancellation policy, or else no refunds will be given:  INITIAL   
• I agree that   Pet Name   is up to date on all shots:   INITIAL   
• I agree to release Company of liability in the event something should happen to   Pet Name :   INITIAL
