HBME is a registered tax paying entity that requires ALL its employees and subcontractors affiliates to pay their portion in taxes. After completing this document, you WILL BE REDIRECTED TO COMPLETE the IRS W-9 form.

After completing the W-9 form, you will be redirected to the HOLD HARMLESS agreement site.

After completing the Hold Harmless agreement, you will be re-directed to complete the Stipend Form. This form is only applicable if HBME has informed you of this agreement AHEAD OF TIME. If not, this form is NOT APPLICABLE to complete. If so, you can then exit the screen after the HH agreement completion.
