Authorization and Permission is hereby granted to SUBR Educational Talent Search program to speak with school administrators, access and collect my child’s records- Transcript, Progress Reports, Class Schedules, and other necessary documents; Financial Aid and College/University admissions documentation and other appropriate assessments as part of SUBR ETS program services, and in accordance with federal rules and regulations. Also, we/(I) allow photographs of my child taken as a participant during program activities, field trips, and projects to be used for Educational Talent Search publications, reports, informational, and educational purposes. We (I) certify that all information herein is to the best of my(our) knowledge and without intent to misrepresent.
