DE OPT OUT OF RECITAL FORM

Dear Parent/Guardian:

By signing this form, you are requesting that your child not participate in the year-end, performance show (recital). You will not be charged for the performance package, however, if you change your mind and your dancer would like to participate in the recital, Dance Expressions must be notified, via e-mail, no later than November 1st. If opting out AFTER November 1st, you will be responsable for the full custome fee.
