Please ensure that you are read through this form carefully and have read my "FAQ's" before submitting so that you are aware of my process and how to prepare yourself for your appointment.  ~ My email response to your submission form will touch base on our project, and will include your appointment date (or an alternative 2 appointment dates/times to choose from if your preferred dates are taken) and answers to the questions you may list below.

This form will be used as the main transfer of information. Be sure you know what it is that you're looking for your tattoo! That being said, I don't mind if we need to talk via email after this to come up with more ideas! ***I only do custom work and prefer to include natural, botanical, astrological, historical, and/or metaphysical design elements. Be aware that you piece will be a custom creation that is unique and one-of-a-kind!
