  • Dear New Patient,

    Welcome to Bayside Pediatric Therapy! Our team is excited to work with you and your child. It is our goal to help your child succeed in all of their developmental and communication goals. We do our best to verify your insurance eligibility and benets beforehand, but ask that you do so as well. All co-pays, co-insurance and balances are due at the time of service. For your convenience we accept cash and credit/debit cards. Please direct any billing questions to our billing department.     The initial evaluation takes approximately 1 hour to complete. You are always welcome to sit in on any sessions with your child. Please do not bring other children to the session or leave them unattended in the waiting room. Please avoid cell phone use during the session as it can be a distraction. Please do not record a therapy session without the therapists permission. Please do not post any therapy videos or pictures on the internet or social media without permission. During the evaluation the therapist will review with you treatment recommendations and frequency of visits. The therapist may assign homework or suggest carryover skills to practice at home. It is imperative to your child's success that these be completed between sessions. If your therapist is out of the oce for any reason on the day of one of your sessions, we may assign another therapist to see your child for continuity of care.

    We are a teaching facility and at times will have graduate students in their last year of school observing and participating. Please know that the student will never be alone treating and will always be under the supervision of a licensed therapist. If you have any questions, please feel free to reach out to us.

  • Attendance Policy

  • Thank you for choosing Bayside Pediatric Therapy. We are committed to not just delivering a therapeutic visit for your child, but truly transforming their milestones and goals. We believe in transparency as it is the key to creating authentic relationships for all we serve. You have a choice in picking your therapeutic provider and we value your choice. With that being said, we want to ensure that you understand the reason why we have a strict attendance policy and why we must hold each patient accountable with no subjective flexibility. Please do not sign this policy unless you are in full agreement and understand that this is non-negotiable.  Failure to follow our attendance policy will result in cancellation charges and may lead to discharge as a result of patient non-compliance.


    Based on our clinical expectations and commitment to delivering clinical outcomes, your child’s attendance and continuity of care are truly critical and essential.  We appreciate you understanding that we are a small clinician owned business and are doing everything we can to support our community. Last minute cancellations not only impact our practice but also impact your child’s progress, as well our clinical team feeling confident in continuing to support your child’s treatment plan. We appreciate your understanding that we are making financial investment to support your schedule and when you cancel in less than 24 hours it creates financial hardship on your therapist and our organization as a whole. As a healthcare provider, we are a fee for service entity and when your cancellation occurs at the last minute, we are not able to fill your appointment slot putting our stability as an organization in jeopardy. Stability of appointments and advanced cancellation notice is a necessity to continue to receive care within our organization


    We want to provide the best possible services to all of our patients. We will do our best to schedule appointments that meet your needs. Regular attendance is a requirement as well as advanced notice of any cancellations or changes in schedule. We ask that you follow the attendance policies outlined below:

     

    1. Less Than 24 Hour Cancellations: Please call us at least 24 hours in advance tocancel your appointment. You MUST notify the front desk, not your therapist. We reserve the right to charge a $50 fee directly to your credit card on file, if you do not give us 24 hours’ notice. Insurance will not cover this fee.If you agree to do teletherapy or a “make-up” appointment is available in the next 4 days, this will be waived.  You are allowed 3 same day cancels within 6 months. Please initial you are in agreement with this cancellation expectation and the financial implications of your actions:_______

  • 2.  Missed Appointments: If you cancel or do not attend 75% of appointments within 2 months, we will put your services on hold until scheduling problems can be worked out.

  • 3. Late for Appointments and No Shows: If you are more than 10 minutes late for your appointment, we reserve the right to cancel the appointment and consider it a missed appointment (see policy for missed appointments above). If you are late for 3 or more sessions, we may put your services on hold until scheduling problems can be worked out. If you no-show for 2 appointmetns within 6 months, we reserve the right to remove you from the schedule.  You can be places on a wait list until scheduling is resolved or the on call list.  Please initial you are in agreement with this late for appointments expectation and implications of your actions: 

  • 4. Clinician Cancellations: If your therapist cancels in less than 24 hours notice and we are not able to fill your slot with a different therapist, then we will credit you with a visit for a future late cancellation that you have. The vision is that we are holding our team to the same cancellation expectations as our patients. Please initial you are in agreement with the clinician cancellation expectation and implications of their actions:
