By signing and submitting this application, I am Volunteering for Happy Tails Pet Sanctuary for a short-term, intermittent or one-time project. While on this project, I agree to abide by the rules and and procedures directed by Happy Tails, Inc. and be and will be respectful of its mission. When I accept volunteer work, I will abide by the schedule and perform the tasks assigned. When volunteering, I will act in a humane manner toward all animals and people with whom I interact. I understand that Happy Tails has a zero tolerance policy regarding discrimination and violence toward people and animals, and agree to conform to this policy.

Volunteer Agreement & Release Form

I fully understand and agree that my services are provided strictly in a VOLUNTEER capacity. I fully understand and agree to assume all risks involved in any and all duties that I perform for Happy Tails in my volunteer capacity. Such duties may include, but are not limited to: animal handling, shelter/cage assistance, custodial work, and other foreseeable volunteer duties. I fully understand that Happy Tails, Inc. expects high standards of moral and ethical treatment of all animals including, but not limited to, the animals under its care. I also understand that Happy Tails has a zero-tolerance policy against any type of harassment, discrimination, violence, threat or intimidation (implied or actual), and against any use of illegal drugs or alcohol. I agree to adhere strictly to these standards and policies. I agree to adhere strictly to these standards in my volunteer capacity. I fully understand and agree to comply with any and all of the obligations outlined in this Volunteer Agreement. For any reason whatsoever, while performing my volunteer services, Happy Tails, Inc. may terminate my services without cause. I hereby agree that I am providing volunteer services to Happy Tails, Inc. which may include handling of animals. This may also include volunteering through Happy Tails, Inc. at third parties or local businesses including, but not limited to, PetSmart stores. I understand that neither Happy Tails, PetSmart nor any third partyibusiness where I volunteer on behalf of Happy Tails, Inc. is responsible for any illness or injury caused by any animals that I come into contact with during my volunteer work. I agree to hold harmless and release from liability Happy Tails, PetSmart, and any other third partyibusiness where I volunteer on behalf of Happy Tails, Inc., should I become sick or injured from any animals as a result of my volunteer work. On behalf of myself, my heirs, personal representatives, executors and administrators or anyone else who might claim on my behalf; I hereby release, discharge, indemnify and hold harmless Happy Tails, Inc., its agents, servants, volunteers and Board of Directors from any and all claims, causes of action or demands of any nature or cause connected with my Volunteer Agreement and service. I allow Happy Tails, Inc. to use any photographs taken of me for use in public relations efforts and hold Happy Tails, Inc. harmless for any and all loss or damage to my personal property while performing volunteer services for Happy Tails Pet Sanctuary.
