Parish Registration & Census Form

A warm welcome is extended to all who come to Blessed Sacrament. As a faith community, we urge that all parishioners register with the parish office. This ensures that we provide you and your family with the proper catechetical, pastoral and sacramental care.
- Certificates of eligibility to sponsor at Baptism or Confirmation will ONLY be provided to registered parishioners who are active and practicing Catholics.
- Only registered parishioners can be enrolled in our religious education program.
- Kindly notify us if you move out of the parish.
