BOARDING VACCINE WAIVER

Alta Vista Boarding & Grooming, LLC, recommend all vaccines be current a minimum of 10 to 14 days prior to boarding. This form is a waiver to acknowledge any unforeseen illness that may surface while your pets are in our care, and you, the pet's owner, do not hold Alta Vista Boarding and Grooming, or its subsidiaries liable. That you, the pet's owner, are responsible for any medical bills incurred.

My Pet Has Received Required Vaccines:

I understand that while my pet/s is boarding at Alta Vista Boarding and Grooming, LLC, that they will be vaccinated for the "no" checked vaccines required. I understand that while vaccines are not 100% effective in every case, they are my pet's best defense to prevent illness. I understand that in some cases, i.e., Bordetella (kennel cough) the vaccines may not have any, or full effect for 7 to 21 days. I understand that my pet may be susceptible to contracting illness by being in close proximity to other pets, even though they have also been vaccinated. I further acknowledge I sign this waiver in agreement to not hold Alta Vista Boarding and Grooming liable for any illness that may arise due to the above vaccines not being current, and that all medical expenses will be solely my responsibility.
