    Plaza Del Amo Animal Hospital is only providing boarding services to our patients that have an up-to-date exam with us. -- We are also currently closed for boarding pick up on Sundays.
  • Vaccines are required as followed: (must be current with valid proof of vaccination date)      

    Dogs: DHPP, Rabies, Bordetella and Canine Influenza
    Cats:  FVRCP and Rabies.

  • Please fill out the following form and click "Submit" when finished before the date of your Check-In to help ensure a more efficient check-in process.

     

    We REQUIRE the boarding form to be filled out at least 24 hours before boarding stay

  • Feeding Information

  • We feed a prescription bland diet while your pet is here to help prevent colitus. However, it is best for your pets not to change foods so we recommend that you bring your pets current food in prelabled and measured baggies.  There is no extra charge for this service.

  • We are happy to give your pet his/her medications for an additional charge of $8.00 per administration.

    Medication must be in ORIGINAL container with prescribed label

     



  • Playtime and Extra Services

  • All pets will recieve a minimum of two 15 minute walks per day along with pets and love during those walks.  

    Additionally, we offer playtime, cuddle and hugs time, brush time and additional walks at an additional cost.

    • Medications: if your pet will be recieving medications during his/her stay it must be in the veterinary-labeled containers with instructions for administration.
    • Vaccine Requirements: In order to protect the health of your pet, this facility requires  documentation(invoice or veterianry medical records) showing that all pets are current on vaccinations.  Pets that are so young that they have not completed their entire series of vaccinations may not yet be protected and, thus, owners accept any risks of infection. ** we do board unprotected puppies in a isolation area to try and keep them protected**

       A treatment will be administered at the owner’s expense to any pet with evidence of fleas, flea dirt, or ticks. 

      Statement of Kennel Policy 

                                                              

      I authorize the administration of vaccines, medication, tests, surgical procedures, use of anesthesia and/or treatments as deemed necessary by the veterinarian and understand that hospital personnel will be employed in the procedure as directed by the veterinarian.

      In the event that my pet develops any symptoms of illness such as vomiting, diarrhea, skipping multiple meals, etc. or we observe parasites, we will examine your pet and treat as needed. We will call the contact number listed to discuss our recommendations before starting any treatments, but if we are unable to reach you we will begin treatment as needed to maintain your pet’s health, comfort, and well-being.

      In the event of an unforeseen emergency, we will attempt to reach you without delay. Please know that we will take every precaution to ensure that your pet is safe.  Very rarely, emergencies do happen and we want to know your preference if no one can be reached.
