Online Coaching Consent

I understand that I have the following rights with respect to virtual Coaching:

1. I have the right to withhold or withdraw consent at any time without affecting my right to future care or treatment nor risking the loss or withdrawal of any program benefits to which I would otherwise be entitled.

2. Confidentiality of my information also apply to coaching. As such, I understand that the information disclosed by me during the course of my coaching is generally confidential. However, there are both mandatory and permissive exceptions to confidentiality, including, but not limited to reporting child, elder, and dependent adult abuse; expressed threats of violence towards an ascertainable victim; and where I make my mental or emotional state an issue in a legal proceeding.

3. I understand that coaching does not replace the need for medical or mental health professionals.

4. Clients can cancel or reschedule an appointment anytime, if they provide 48 hours’ notice. If you cancel an appointment with less than 48 hours’ notice, or fail to show up, you will be charged the full rate of service. An exception to this rule may be changing your session to another open slot in the same week, however open slots are not guaranteed to be available.

Use of Session Recordings

From time to time, Groundwork Academy records coaching sessions for quality, education, and training purposes. With your consent, recordings may be used in the following ways:• Internal training and education for coaches who are undergoing Groundwork Academy training• Content creation where only your coach is visible and/or audible (for example, educational clips, reflections, or teachings inspired by sessions) Client identity will NEVER be revealed in public-facing content. Your name, image, voice, and any identifying details will not be shared on social media, websites, or marketing materials.
I consent to sessions being recorded and used for internal training purposes only within Groundwork Academy with Groundwork-trained coaches.I consent to sessions being recorded and for excerpts to be used in educational or social media content where only Madison is visible and/or speaking, and my identity is fully protected.I do not consent to being recorded for any reason
