    This instrument is for your own use. We request you to kindly be honest with your responses. Read each statement given below and indicate on the right-hand blank space how much it is true of your behavior, by using the following guidelines:

    0 = not at all characteristic of you or you normally do not do this (nearly 0 or than 10%)

    1 = Not characteristic of you and you may do this rarely ( less than 25%)

    2 = Somewhat characteristic of you and you do this sometimes (50%)

    3 = Fairly characteristic of you and you do this most of the time (75%)

    4 = Highly characteristic of you and you do this almost always (90% and above)
