  • If you make a purchase using your personal account and want it to be reimbursed from your scholarship, you can submit a request by uploading the transaction receipt below. You may also use this form to access funds that are left over from previous terms. If your request is greater than your available balance, you will be funded the maximum possible amount. 

  • Please upload receipt(s), budget, or other documentation below for the funds you are requesting.

    If this is a reimbursement, your receipt must include the following:

    • Transaction details with items, date, and vendor/institution name
    • The total amount of the transaction

  • Please submit your files as PDFs only. This would be best accomplished using a desktop or laptop computer. Most of the documents we request can be saved directly as PDFs. If not, you have the ability to manually save any webpage as a PDF (click here for instructions). If needed, you can also convert an image (such as a screenshot) to a PDF here.

  • Transcript

  • Your transcript must include the following:

    • Your name
    • Your GPA for the latest academic term
    • The courses you took in the latest academic term
    • Your cumulative GPA

    You may need to submit multiple pages to capture all of the above information.
