By filling in the form here, you agree to disclose your personal contact (both contact number and email address) to the organising committee for their follow-up / necessary contact for this project.

家長需要完成繳費程序 (以支票形式繳付總金額) 才算正式完成登記手續。 未於期限繳費者，視同未完成報名手續。請把支票放進信封，在信封上寫上園生姓名及就讀班別，在2021年1月29日或之前交回班主任。

Registration is not complete until the cheque payment for the full amount is made and confirmed. Please submit your cheque in a sealed envelope, with your child’s class and name written on the envelope, to your class teachers by 29th January, 2021. 

In the event that we cannot cover the cost of this project, we will cancel production and all money raised will be
refunded to you (after deducting for expenses). Any surplus amounts raised will be used towards graduation gifts or donated to the
PTA or a registered charity. Save and except the aforesaid, please note that fees will not be refunded.

此紀念 冊能否成功出版實有賴各位家長的大力支持。如數量未達最低要求，本會將取消此項計劃，扣除開支後把款項退回，敬請諒解。畢業生紀念冊之製作盈餘(如適用)將用於畢業禮物或捐贈給家教會或慈善機構。​​請注意，除前述的情況外，費用將不獲退還。
