St. Joseph’s Home Care is committed to promoting a safe and healthy workplace environment for its employees (including students), visitors and contractors. Prevention of workplace injuries, illnesses and incidents is a key component of creating a safe work place.
The Occupational Health and Safety Act and the Workplace Safety Insurance Act provide regulations for all employers to follow with respect to injury and illness investigation and reporting.
It is important to remember that reporting of all injuries, illnesses, near misses and work refusals allows SJHC to find solutions to prevent future incidents and to keep workers and clients safe.
Any employee that has a work-related injury, illness, incident, near miss or work refusal are required to complete the following steps:
(1) Obtain medical treatment (e.g. medical aid, first aid, 9-1-1), if required.
(2) Report immediately to your direct manager/supervisor.
(3) Obtain this tool kit from your direct manager/supervisor.
(4) Complete pages 1 and 2 of this toolkit within 24 hours of the work-related injury, illness, incident, near miss or work refusal and submit to your direct manager/supervisor.
(5) If you seek medical attention advise your direct manager/supervisor immediately and you will be required to submit a copy of the WSIB Form 8 – Physician’s Report and complete the WSIB Form 6 – Worker’s Report of Injury/Illness. Please keep originals of both forms for your files and send copies to the OHS Coordinator within 24 hours.
(6) Needle stick - If your injury is a result of a needle stick, please refer to procedure OHS 21-05 (request from your direct manager/supervisor).
For managers/supervisors please complete the following steps:
(1) Make sure that the employee has sought the proper medical attention (if required).
(2) Remind the employee to complete pages 1 and 2 of the toolkit within 24 hours of the injury, illness, incident, near miss or work refusal. Follow up with the employee if report has not been received within 24 hours.
(3) Manager/Supervisor to review pages 1 and 2 making sure all areas are complete.
(4) Manager/Supervisor to complete all sections on page 3 (Incident Investigation Form) by following up with the employee.
(5) If required, manager/supervisor will initiate the return to work program.
(6) All completed pages are to be submitted to the OHS Coordinator within 24 hours of the injury, illness, incident, near miss or work refusal. If there is need for a longer investigation, the manager/supervisor is to complete as much of the form as possible and forward to OHS Coordinator and advise of the continued investigation. Once report is complete, forward to OHS Coordinator.
OHS Coordinator
(1) Once the completed Injury, Illness and Incident Report & Investigation Forms have been received, OHS will review and will follow up with the manager/supervisor where required.
(2) If applicable, OHS will complete the WSIB Form 7 within three days of the date of the injury or illness and will initiate the return to work program immediately with the manager/supervisor.
Any student, visitor or contractor who sustains an injury, illness, incident or near miss at any of the SJHC facilities must complete this report immediately and submit to the manager/supervisor of the facility.
