Important Information
1. Appointments need to be cancelled at least 24 hours before the session. Sessions not canceled in time will need to be charged.
2. Payments can be made by Bank Transfer (please send proof of payment), online card payment, or cash on hand before or on the day of the appointment.
3. The client may rest assured that session content is strictly confidential. If, however, the client is considered to be a danger to themself or to someone else, the necessary parties will be contacted with the client's knowledge.
4. Due to the nature of psychological interventions, arriving and ending appointments on time are essential to good therapeutic practise.
5. A standard consultation is 50 minutes (plus 5-10 minutes administration time) in length at $180 (all inclusive) per session.
6. Office hours are Mondays to Saturdays 09:00 to 15:00. It is preferred that the client-psychologist contact be reserved for sessions, however, the psychologist may be contacted during these hours, preferably via text or email.
7. In the case of a mental health emergency, please contact the nearest Emergency Rooms. The psychologist may be notified at the client's or next-of-kin's soonest convenience.
8. Signing below means that you agree to the information above, and that the client and/or parent/caregiver take full responsibility for their account with the practice.
9. By signing this form, you agree that you have permission from the other parent or caregiver to consult at the practice.
