Music Beans - Registration Form

1. New student: + $30 registration fee (one-time only)

2. Registration is ongoing with prorated tuition.

1. All the requests for withdrawal must be received in writing by Music Beans.

2. Tuition is NON-REFUNDABLE after the second class of the registered semester.

3. A tuition credit or refund for unused lessons is available minus an $80 withdrawal fee.

4. Students are responsible for the tuition of any lessons within a two-week notice period.

5. The registration and administration fees are not refundable.

1. In order to have a makeup lesson scheduled, please notify the office 24 hours BEFORE your absence. A make-up lesson will be scheduled at a time that is mutually convenient.

2. 3 make-up lessons allowed in the 2020 Fall semester.

3. All the makeup lessons need to be COMPLETED by the end of the registered semester.

4. Methods of make-up lessons:

(1) add 15min to the following consecutive classes.

(2) schedule another mutually convenient day for a make-up lesson

(3) virtual lessons may be arranged a on a case-by-case basis.

1. We hereby authorize Music Beans (“MB”) to take emergency procedures/actions MB deems appropriate in the event a child is ever left unattended while participating in the program. We further waive claims against MB with respect to the same. We further represent that neither us nor our child will be ill when coming to Music Beans and the child will not, to the best of our knowledge, have been exposed to any contagious disease.

2. We agree not to directly or indirectly solicit, recruit, attempt to hire or recruit, or raid the personnel of MB or otherwise induce the separation of any personnel from MB.

3. We agree that my children may be videotaped, filmed, or photographed at Music Beans school-related functions and during class activities. We grant permission to MB to use the materials above on its official website, social media, or in other official school printed publications. MB agrees to will not sell any of the above materials to a third party.

·  I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. 

 

· I acknowledge that Music Beans LLC (MB) has put in place preventative measures to reduce the spread of the COVID-19. MB cannot guarantee that my child(ren), caregivers, my family members, and I will not become infected with COVID-19 while attending classes on MB premises and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 while on MB premises may result from the actions, omissions, or negligence of myself and others, including, but not limited to, MB faculty, program participants and their families and/or any other individual who may be present upon school property or in attendance at any school activity.

 

· I acknowledge that I must comply with all set procedures by New York State and New York City to reduce the spread while attending class at Music Beans. I attest that I, my child(ren), my family members, and my caregivers of child(ren):

1) do not experience any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

2) have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as noncontagious by state or local public health authorities.

3) have followed the New York State and New York City on travel and quarantine guidelines. 

I voluntarily agree to assume, on behalf of myself, my child(ren), my family members, and the caregivers of child(ren), all risks and accept sole responsibility for any injury (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I, my child(ren) and/or members of my family may experience or incur in connection with my child(ren)’s attendance and participation in Music Beans LLC. I hereby release and agree to hold Music Beans LLC harmless from, and waive on behalf of myself, my child(ren), my family members, and my caregivers of child(ren) any and all causes of action claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of MB. This liability waiver and release extends to the school together with all owners, partners, school faculty, and staff.
