  • Eligibility as Test Administrator/Proctor

    To be a test administrator/proctor administering CASLI exam, I understand that I MUST be either already RID Certified OR have NO plan to pursue RID Certification taking CASLI exams. 

    Once one becomes a LTA test administrator/proctor, they will not be able to pursue RID certification include taking any CASLI exam for at least 5 years after they have ceased as test administrator/proctor.

    IF you plan to pursue RID Certification taking CASLI exam in the near future, please STOP HERE and communicate with your LTA site coordinator and CASLI immediately.

  • Test Administrator / Proctor Contract

    Please read, initial and sign below.
  • This is a binding contract between Center for the Assessment of Sign Language Interpretation (CASLI) and you, as a contractor as a test administrator/proctor. As you are a test administrator/proctor to a designated Local Testing Administrator (LTA) site, there are specific expectations and procotols in places that test administrators/proctors needs to be aware of prior to administering any exams.

  • Responsibilities as Test Administrator / Proctor

    • Provide a professional proctoring service to all exam candidates.
    • Ensure the security of the examination and all exam-related materials, following CASLI policies and procedures as outlined in CASLI's Test Administrator Training Manual.
    • Ensure testing computer is operational and functional ahead of time prior to administration of each exam.
    • Verify candidate's ID prior to administering exam.
    • Provide an appropriate testing environment for candidates to take their exam in. 
    • Complete and report any issues or incidents that occurs prior, during and after exam administration including when candidates are no call/no show for their exams.
    • Review and follow all of the protocols outlined in the CASLI Test Administrator Training Manual.
    • Complete and submit Proctor Payment Invoice in a timely manner. If the invoice is not received within 30 days, it will automatically be considered as in-kind donation. 
    • Contact CASLI with questions or concerns.

     

  • Exam Accommodations:

    From time to time, we have candidates who request ADA Accommodations such as extended testing time, frequent breaks, ability to sit or stand, bottled water in the exam room, etc..

    By your initals below, you acknowledge that in the event a candidate requests ADA Accommodations, you will be contacted by CASLI with guidance on the provision of these accommodations and as a test administrator/proctor you are obliged to provide those accommodations to the best of your ability.

  • Payment Invoice for Exam Administered:

    Proctor Payment Invoice, available on CASLI Exam System, for exam administered must be completed and submitted within 5 business days of the end of the month.  LTAs should consolidate and list all testing activity from the previous month on one comprehensive invoice submission. The submission of multiple invoices in a month is discouraged. Failure to submit an invoice within 30 days will be considered as a defacto in-kind donation of services to CASLI.

  • Confidentiality, Ownership, and Non-Disclosure Agreement

  • I acknowledge that in connection with my service as a CONTRACTOR with the Center for the Assessment of Sign Language Interpretation, LLC (CASLI) on the national, regional, state, and/or local level, I will have access to and may become aware of confidential information, including, without limitation, exam content, information regarding procedures, methods, and data used by CASLI in the assessment of interpreters throughout the country.  I further acknowledge that any information that is shared with me or accessed by me during my service with CASLI that is identified as “confidential” or that has not been made publicly available by CASLI is deemed to be of a confidential nature and is owned as a proprietary right by CASLI.


    I agree that I will not allow any third party to have access to any materials containing such confidential information provided to me by or on behalf CASLI; that, upon CASLI’s request or upon the completion of my service with CASLI, I will delete, destroy, or return to CASLI any such materials in my possession; and that I will not reproduce any such materials by any means, including memorization or recall.  


    I further agree that I will not use such confidential information for any purpose other than the scope of work assigned to me as a CONTRACTOR, or for consultation purposes, and will not disclose this information to any third party during my consultation or service with CASLI or subsequent thereto, unless otherwise authorized by CASLI in writing.  


    I acknowledge that CASLI may pursue legal action for any violation of this Agreement or other improper use of such confidential information by me or by others through me, and that CASLI, in connection with such action, may use this Agreement, and may seek all available remedies, including without limitation, injunctive relief, damages, and attorney’s fees and costs.  

  • Conflict of Interest

  • Outside Business/Professional Relationships with CASLI examination candidates: 

  • Any Other Actual or Potential Apparent Conflict(s) of Interest: 

  • I understand that, if during my time of service, a new conflict of interest or an appearance of conflict should arise, I must notify CASLI in writing and within 30 days of such occurrence. I understand that an actual or apparent conflict of interest may disqualify me from volunteer service or continued volunteer service with CASLI.

  • Payment Information

  • As a test administrator/proctor for a designated Local Test Administrator (LTA) site for CASLI exams, you understand that compensation for exam administration may be designated as payable to either the organization or to individual LTAs. Both individuals and organizations receiving these direct payments will receive form 1099s for tax reporting purposes annually, and must submit a completed. 

    To properly set the test site up as a vendor for CASLI, please use the following link to submit a W-9 form for the legal entity (the organization or the individual LTA) that will be receiving the exam administration payment, even if such payments will be designated as in-kind donations.

  • Direct Deposit Payment:

    If you will be receiving payment directly or are arranging payment to be sent to your site and wish to receive the payment through direct deposit, please complete the Vendor Direct Deposit Enrollment Form.

  • ACKNOWLEDGMENT

    This contract is good for TWO years from the date of signing this contract and shall be renewed again to continue role as test administrator/ proctor. Any changes must be communicated to CASLI immediately via email to manager@casli.org. 

    Termination Without Cause: CASLI may terminate this contract without cause at any time.

    At any times there are questions or concerns, contractor is responsible to contact CASLI for answers or resolutions to concerns.  

    With my signature below, this acknowledge that I understand the contract and what is expected of me as a contracted test administrator/proctor. Furthermore, I agree to the foregoing and I certify that the information I have provided in this form is true and complete to the best of my knowledge.
