Each applicant to LiNC agrees to release all parties involved in LiNC, including you, from any liability that might result from the information you provide here. Applicants also waive any right to inspect or review references given on their behalf.

We know that your time is valuable, and we have worked to keep this form as short as possible. As you answer questions, we hope and ask that you take time to answer as carefully, thoughtfully and objectively as you can.
