Family Cookie Responsibility Form

Each participating Girl Scout family must read and acknowledge understanding of the adult/family responsibilities and cookie sale procedures prior to participating in the 2025 Girl Scout Cookie Program, including cookie booths. You will be asked to provide your troop leader's or cookie manager's email address in order to notify them of completion. Forms need to be completed by February 16, 2025. Girl Scouts may store cookie inventory with separate parent(s)/guardian(s), but each parent/guardian will need to sign a Family Cookie Responsibility Form and have it on file with the troop.
