• Enrollment Agreement

  • Faith Christian Preschool operates on a carefully controlled budget. An integral part of maintaining that budget is the timely receipt of all registration, tuition, before & after school care, lunch and other fees. In order to accomplish this goal, the FCP/Church Board has adopted the following tuition payment policy. This policy will be STRICTLY ADHERED TO and enforced by the Administration.

    The financial agreement is a document explaining the financial obligations for enrollment. At any time, if a family decides to withdraw from the school, the outstanding charges will not be cancelled. Final clearance will be complete once all financial obligations have been fulfilled to the satisfaction of the Administration.

    Tuition payments will be made through your Brightwheel. Please see our Tuition Payments information sheet for more information about tuition payments.

    All accounts must be current for families to register for the upcoming year.

  • Application Processing Fee

    Per new family: Non-Refundable
  • The Application Processing Fee is required at the time of application to FCP. This fee covers some of the cost associated with the application process and the registration costs. This fee is per student and is non-refundable.

  • Registration Fee

    Per student: Non-Refundable
  • The Registration Fee is required at the time ALL students complete the registration process. This fee is a per student fee and is non-refundable.

  • Curriculum Fee

    Per Student: Non-Refundable
  • The curriculum fee is required for students enrolling in K2 & K3 classes at the time a new students completes the registration process & returning students complete the re-registration process. This fee is a per student fee & is non-refundable.

  • Entrance Screening/Interview

  • An entrance screening is administered for all students entering the preschool program.

  • Late Accounts

  • In order to maintain a high quality of education for each child, it is necessary that all tuition payments are made on Friday for the upcoming week and will be made on a weekly basis. Payments will be made electronically via mybrightwheel.com. Our teachers are hired on the assumption that each child will be in school: when he/she is absent payment must be received for this day as our expenses continue when the child is or is not present. In order to be fair to the parents & to protect our own financial structure, the FINANCIAL AGREEMENT that you signed upon registering your child is final.

    Tuition payments after Monday would have a late fee of $10.00. If payment is not received by Monday afternoon, the student will have a NO ADMIT on Tuesday.

    PLEASE NOTE: Tuition & curriculum fees must be current in order for a student to attend the first day of class. Should a student be withdrawn from FCP for any reason, application, registration, & curriculum fees are non-refundable in whole or in part.

  • Late Pick Up

  • A $15 late pick up fee will be charged for the first minute and $1 each additional minute & is expected to be paid upon arrival when the student is picked up.

  • Tardies

  • A $5 tardy fee will be charged for any student arriving at 9:01 am or later unless a doctor's note is presented.

  • Returning Families

  • Returning families registering for the upcoming year must have their accounts fully current to be eligible to re-register.

    Registration will not be accepted if there is an unpaid balance on an account. Also, if a balance is incurred between current school year and new school year, the student(s) will not be allowed to attend until account is current.

  • TERMS & CONDITIONS

    • I agree to pay an application & registration fee at the time of enrollment.
    • I agree my child will participate in the education program for which he/she is eligible & to pay the annual curriculum/supply fee where applicable.
    • I agree to pay the weekly tuition fee. I understand that there will be no deductions for sickness, absence, or holidays. A late fee is assessed if tuition is not paid by the end of business on Tuesdays, or if absent, by the end of the first day attendance.
    • I agree to pay the full weekly tuition fee even if my child is absent for one or more days during the calendar week (defined as Monday through Friday).
    • I understand that if I am late for a payment, my account will be charged a late fee.
    • None of the above fees identified, including curriculum, program, registration, & field trip fees are refundable.
    • Faith Christian Preschool is open Monday through Friday, except on FCP holidays and other designated days on the school calendar.
    • I agree to pay a per child late pickup fee, for each period of time after FCP's closing as indicated on the fee schedule in effect.
    • I agree to pay an insufficient funds fee for refused ACH transactions.
    • In case of withdrawal of my child from FCP, I agree to give FCP written notice two weeks prior to withdrawal.
    • Legal authorities may be contacted for children left at FCP for more than one hour after closing time.
    • The terms of this agreement may be subject to change in whole or part by Faith Christian Preschool. A reasonable attempt will be made to contact each family by phone, email, and/or letter.
    • Faith Christian Preschool may terminate this agreement at any time. A child may be asked to withdraw by FCP without prior notice if, in the sole opinion of FCP, it is in the best interest of the child or FCP to withdraw the child.
    • I agree to pay a late pick up fee per child according to the fee schedule in effect if my child is still present in the building after 6 pm, regardless of whether or not a parent/guardian is present.
    • Faith Christian Preschool, its owners, directors, & employees or associated companies are NOT responsible for reimbursement of any medical expenses incurred as a result of accidental incidents to a child or incidents between children resulting in injuries that occur to a child or children during attendance at Faith Christian Preschool, whether or not under the supervision of any Faith Christian Preschool employee.

  • FAMILY HISTORY/MEDICAL EMERGENCY TREAMENT AGREEMENT

  • I/We , residing at are the parents/guardians and have legal custody of         , a minor, age      in the city of    , zip     County of      State of    born on   Pick a Date   

  • In case of emergency, I/we authorize any representative of FCP to present above stated minor to a medical treatment center, and do consent to an x-ray, exam, anesthetic, medical or surgical diagnosis or treatment, and hospital care to be rendered to the minor under the general practitioner or surgeon licensed to practice in any state of the United States. I/We hereby consent to the same treatment for dental diagnosis or treatment by a dentist licensed to practice in any state in the United States. I/We understand that I/We shall be fully responsible for, and agree to pay for, all costs and expenses incurred in connection with such medical services rendered to my child pursuant to this authorization.

  • FCP does not administer medicine, with the exception of medicines taken by Nebulizers and inhalers.

    • Disbursement of these medications is as follows:
    • Completion of an Authorization Form
    • Medication in the ORIGINAL container

    All meds must be delivered by the parent(s)/guardian(s) to the Office. Meds should never be in the possession of the child, in backpack, lunch box, or on person, etc.

    You will be contacted if your child exhibits an elevated temperature (100° plus), if a medical emergency develops, if an incident requires emergency care, if your child is vomiting, or continued visits for the same complaints warrant a student being sent home from school.

  • Health/Illness Policy

  • A child who becomes ill during the day will be monitored for a period of time by a staff member.

    IF SYMPTOMS PERSIST THE PARENT WILL BE CONTACTED TO PICK UP THE CHILD.

    Any child suspected of having a communicable disease or contagious condition shall be placed in a designated isolation area. The condition shall be reported to the parent/guardian and the child shall be removed from the facility as soon as possible.

  • Symptoms that may require contacting parent/guardian:

  • COMMUNICABLE DISEASES

  • Children shall not return to the childcare facility until the symptoms of communicable illness are no longer present and a written statement from the physician attesting to the fact that the child has been appropriately treated as provided.

  • ADMITTANCE TO THE CHILDCARE FACILITY FOR THE FOLLOWING COMMUNICABLE DISEASES SHALL BE:

  •  In order to return to school, the child should:

    • Be on prescription medication for 24 hours and/or provide a note from the physician (if appropriate)
    • Be symptom free
      • Temperature below 100.4° F (no fever) without the aid of Tylenol or any fever reducing substances
  • DISCIPLINE POLICY

  • BITING

  • The policy for habitual biting is as follows:

    1. The first time a child bites another child, the parent will be called and informed, but not necessarily required to remove the child.
    2. Any child that bites twice in two weeks or in a period of 10 days MUST BE PICKED UP IMMEDIATELY and CANNOT RETURN FOR THE NEXT TWO DAYS.
    3. If after the one day suspension, a child bites again, or for the 4th time, the child MUST BE PICKED UP IMMEDIATELY and CANNOT RETURN FOR THE NEXT TWO DAYS.
    4. If after the two day suspension the child bites again, or for the 5th time, he or she will be WITHDRAWN FROM FCP.

    *Registration and tuition paid will not be refunded if a child is permanetly suspended from FCP for biting.

  • AGGRESSIVE BEHAVIOR

    Defined as, but not limited to: hitting, kicking, swearing, tipping over chairs, etc.
  • The policy for a child who repeatedly interrupts class with aggressive behavior is as follows:

    1. The first time a child displays any behavior similar to the above listed, the parent will be called and informed, but not necessarily required to remove the child.
    2. Any child that becomes aggressive twice in two weeks or in a period of 10 days MUST BE PICKED UP IMMEDIATELY and CANNOT RETURN THE FOLLOWING DAY.
    3. If after the one day suspension, a child becomes aggressive again, or for the 4th time, the child MUST BE PICKED UP IMMEDIATELY and CANNOT RETURN FOR THE NEXT TWO DAYS.
    4. If after the two day suspension the child is aggressive again, for the 5th time, he or she will be WITHDRAWN FROM FCP.

    *Registration and tuition paid will not be refunded if a child is permanently suspended from fcp for aggressive behavior.

     

  • Inappropriate Behavior

    Defined as, but not limited to: exposing oneself, touching someone else inappropriately, etc.
  • The policy for inappropriate behavior is as follows:

    1. The first time a child displays any behavior similar to the above listed, the parent will be called and informed, but not necessarily required to remove the child.
    2. Any child that becomes aggressive twice in two weeks or in a period of 10 days MUST BE PICKED UP IMMEDIATELY and CANNOT RETURN THE FOLLOWING DAY.
    3. If after the one day suspension, a child becomes aggressive again, or for the 4th time, the child MUST BE PICKED UP IMMEDIATELY and CANNOT RETURN FOR THE NEXT TWO DAYS.
    4. If after the two day suspension the child is aggressive again, for the 5th time, he or she will be WITHDRAWN FROM FCP.

    *Registration and tuition paid will not be refunded if a child is permanently suspended from fcp for aggressive behavior.

  • FCP reserves the right to immediately dismiss any child that possess a safety concern to other children in the classroom, regardless of the steps that were taken as outlined in the Aggressive Behavior Policy, Biting Policy, or Inappropriate Behavior Policy.

    If a parent has a disagreement with any decision involving the child, the parent is required to meet with the Preschool Director to discuss their concerns. If a common understanding cannot be reached, the decision by the Preschool Director will remain final. If after reasonable attempts to resolve the issue has not provided productive, the parent and child(ren) may be asked to withdraw from FCP.

  • FCP reserves the right to make the final decision.

    Registration and tuition paid will not be refunded if a child is asked to be withdrawn from FCP for aggressive behavior, biting, or inappropriate behavior.

  • INFLUENZA VIRUS PAMPHLET

    During the 2009 legislative session, a new law was passed that requires child care facilities, family day care homes and large family child care homes provide parents with information detailing the causes, symptoms, and transmission of the influenza virus (the flu) every year during August and September.
