2021 Give a Gift Sign-up Form

Here at Maya's Hope we wanted to give the kids a chance to receive some holiday cheer directly from you. Just let us know your information down below, we will make the necessary happen to find out EXACTLY what your child would like!

We'll send you a photo of the child once they receive YOUR gift.

Gifts requested by the families can be formula, clothing, shoes, games, toys - anything that is age-appropriate and brings joy for the holidays!

We are ordering directly within country. We can order in the Philippines or Ukraine. The donation required is $30 USD to cover your gift and shipping. Donation will be made through Donorbox upon submission of this form. If you wish to give more than $30, the balance will be pooled/used to provide a gift to another child. We will ask the kids what they would like to receive!
