Dear Parent/Guardian:
We are sending you this consent form to inform you about, and request permission for, your mid-schooler or teen to attend a youth development program called PREP (Personal Responsibility Education Program). This program is provided by Amoskeag Health and will take place at Amoskeag Health, your child’s school, virtually, or at a youth program location within your community!
Trained Adolescent Health Educators from Amoskeag Health provide participants with evidence-based, thorough information that encourages them to make choices that can support their long-term personal health goals. In addition to providing the tools to empower teens to make healthy and safe decisions, it also teaches methods to prevent HIV, other sexually transmitted diseases (STDs), and unplanned pregnancy. Some of the topics discussed are:
- Abstinence
- Contraceptive Options
- Healthy and Unhealthy Relationships
- Identity and Gender Identity
- Communication Skills
- Decision-Making Skills
- Contraceptive Options
Teens who complete the program will earn a $50 Amazon Gift Card, the possibility of earning school ELO credit through the Manchester School District, and most importantly knowledge that they can use in their everyday lives!
