I understand that I am responsible for returning the equipment listed above in the condition in which it was received and I willingly take responsibility to make certain that it is returned in the same condition.  I accept financial responsibility for any repairs or replacement of lost or damaged items.

I further understand that I am responsible for ensuring residents have access to the client use equipment for approved activity and that I am to ensure it is stowed in secure location and charged at the end of each day.

I further agree and understand that this equipment is the property of Life Changes, Inc. and is not a gift to me personally.
