    Immaculate Heart and Our Lady of Perpetual Help Confirmation 2026 Application Form

  • The Candidate and Parent must fill this out together. We recommend using a computer/laptop to fill out this form as a phone may not work. Remember you need your Baptism Certificate, Your Essay Video and a Profile picture to upload to submit the application.

    Please note: Candidates and Parents will be contacted with more details in January 2026.  Preparation Classes  will be held on Thursday nights from 6:30 to 8:30 pm at Our Lady of Perpetual Church Hall the choice for what Parish the Candidate will be Confirmed at will be in this form.

  • The Confirmation Candidate will need to record a short video (1-2 minutes think 'Tik Tok' or 'Reels' style (but with you speaking and not dancing)) in response to one of these two questions listed below.

    This video will not be shared via social media or with other Candidates. This video is intended to have the Candidate take a look at their current relationship with God and their Catholic Faith to help them better understand why they are choosing to say YES to Confirmation.

    These videos give us an idea of what topics to focus on during the preparation classes in order to help the Candidate on their journey.

  • Upload copy of Baptism Certificate ONLY if the Candidate was NOT Baptist at Immaculate Heart/OLPH

    or

    did NOT make their First Communion at Immaculate Heart/OLPH.

    If you do not have a copy of the certificate the easiest way to get it is to contact the Parish that your Child was baptized at and ask them to Fax it to OLPH Church.

  • Consent and Liability Waiver for In-Person or Virtual Meetings:

    1. I acknowledge and agree that I remain legally responsible for any personal action(s) taken by the Participant, and I fully indemnify and save harmless the Diocese of London, its Bishop, parishes, officers, directors, priests, deacons, volunteers, employees, and agents including chaperones or other representatives associated with the Event, from any claim for damages to person or property caused by the Participant.

    2. Please note that an adult must be present during an online class where video and sound will be shared with other students in the formation program. As a parent/guardian I give permission for my child to participate in online classes where sound/video images of my child will be shared with other online participants.

    5. I give my permission and consent that my child may use an online video platform to participate in the above-mentioned Event. I understand that my child will not be required to register to a video conferencing platform in order to participate in the program.

    6. I confirm that during all electronic sessions I or a designated adult, are responsible for the supervision of the Participant. For Participants under the age of 12, I or a designated adult will be present. All persons, participants, and adult leaders will be in high-traffic areas.

    By signing, I consent to the provisions of the Electronic Commerce Act, and I agree that an electronic copy of this document, including a pdf, fax, or other electronic attachment, and/or a copy with my signature or electronic signature(s), shall be equivalent to an original signed by myself under seal. By signing this legal document, I am giving up certain legal rights, including the right to sue in certain circumstances. By signing, I confirm having read all items carefully. I am in compliance with the above statements.

  • Authorization for Medical Treatment (when applicable)

    I/we hereby warrant that to the best of my/our knowledge and belief, the Participant is in good health, and I/we assume all responsibility for the health of the Participant.

    I/we hereby grant my/our permission for the Diocese of London to make arrangements for medical attention for the Participant without my/our prior approval if emergency treatment is reasonably required and confirm that I/we will be notified as quickly as quickly as possible if this authority is exercised.   • Media Release:
    Photos and/or videos may be taken to be used on the Diocese of London websites or social media platforms, in printed materials, or submitted with press releases to local publications to illustrate our community for potential visitors. Reasonable efforts will be made by the Diocese of London not to identify participants by name. The Diocese of London is not responsible if
    participants disseminate names.


    Authorization for Communication:
    The Diocese of London may provide resources and communications to further develop your family’s understanding of future activities and events, delivered to the Participant’s school, home mailing address, and/or electronically. I/we consent to receiving resources and communications from the Diocese of London to any of the above-mentioned contact methods. I/we understand that I/we may withdraw this consent at any time.

  • To help cover the cost of the learning materials that will be provided, we ask that a fee of $45.00 be submitted along with your registration form (please, let us know if this is a financial hardship and we will cover the cost of the materials).
