The Candidate and Parent must fill this out together. We recommend using a computer/laptop to fill out this form as a phone may not work. Remember you need your Baptism Certificate, Your Essay Video and a Profile picture to upload to submit the application.

Please note: Candidates and Parents will be contacted with more details in January 2025. Preparation Classes will be held on Thursday nights from 6:30 to 8:30 pm at Our Lady of Perpetual Church Hall the choice for what Parish the Candidate will be Confirmed at will be in this form.

I will send an e-Transfer for $45 to olph@dol.ca with Candidates name and Confirmation 2025 in the memo line (no password needed).I will be placing an Envelope clearly labelled with "Candidates name for Confirmation 2025" in the Offertory Basket at a Weekend Mass.I will be mailing a cheque/money order to Our Lady of Perpetual Help Parish with "Candidates name for Confirmation 2025" (Attention Christine Toffolon.)I would like pay at the first preparation class.I would like to discuss financial assistance.
