The Respite Voucher Program at Aviv Older Adult Services of Jewish Family & Career Services (JF&CS) offers temporary supports for care recipients in order to provide a brief period of relief or rest for family caregivers through the provision of in-home services or adult day care. This Agreement will provide you with important information regarding program requirements.
1. The Respite Voucher Program is a grant funded program that operates through June 2024.
2. JF&CS staff will administer a needs-based assessment over the phone with family caregivers/family representative to determine eligibility for respite assistance.
3. Family Caregivers/Family Representative must be forthcoming about the care recipient’s (loved one) level of need/care. Care Recipient must be weight-bearing and be able to assist with all transfers.
4. While funds are available, family caregivers/family representatives are eligible to make requests for funding throughout the grant period. For extenuating or emergency situations, JF&CS staff may award additional funding. Funds are provided directly to respite providers for services rendered and paid directly to them. The amount awarded is based on need and the availability of funds.
5. Funds can only be used during the approved timeframe. If funds approved are not used within the timeframe determined, family caregivers/family representative must notify JF&CS staff. Based on availability of funds and need, family caregivers/family representative can again at a future date request additional funds.
6. JF&CS contracts with licensed providers of in-home services and adult day centers. Caregivers/family representative can either identify their own providers or receive assistance from JF&CS staff to determine a provider.
7. JF&CS agrees to pay a rate of $30 per hour (in-home services) or daily rate of $100 (adult day care services) & $100 (assessment fee for ADH whenapplicable) with a 4 hour minimum number of hours (in-home services & ADH Services) required per service date.
- This rate is the same even for services provided during holidays and weekends
- JF&CS will only pay for time/services that are actually provided to the care recipient (loved one)
- Family Caregivers/Representatives, and/or Care Recipient (personal care only) are responsible for payment of services in the folowing situations:
- Cancel last minute
- Care is refused
- Agency Caregiver/Agency Staff are allowed to leave early (before end of scheduled shift)
- Payment must be received by the agency within 48 hours of the missed shift. If not, the family will be discharged from The ARC Respite Voucher Program.
- JF&CS does not reimburse providers for mileage incurred by the provider or for any additional costs other than the payment for allowed hours of service
- Invoices for in-home services must be accompanied by telephony records or weekly timesheets signed by the Family Caregiver/ Family Representative and the agency
- JF&CS will provide payment to the respite provider within 45-60 days following submission
- Invoices Adult Day Health services must be accompanied by weekly attendance logs signed by the Family Caregiver/Family Representative
- For ADH services: In the event the care recipient (loved one) uses rideshare (Uber, Lyft, etc..), MARTA, and/or other modes of transportation, the agency must provide the Family caregiver/family representative with attendance logs to be signed and returned to the agency
8. If the family caregiver/family representative exceeds his/her approved hours, JF&CS is not financially responsible for those hours.
9. The caregiver/family representative must notify JF&CS staff immediately if the care recipient is admitted to the hospital or moves to a personal care home, assisted living community, memory care community, continuum of care retirement community (CCRC), short-term rehab, or skilled nursing facility/nursing home. Funds from the Respite Voucher Program cannot be used while the care recipient (loved one) resides in any of these settings. Services cannot be placed on hold for vacations or family visits. Caregiver/family representative must notify the agency and Mandy Rappoport as soon as possible, with any changes in utilizing services.
10. As per The Atlanta Regional Commission, JF&CS and The Respite Voucher Program must meet its contract deliverables and responsibilities to its vendors. While respite is designed to support the ADL and IADL’s it is not designed to support chore services. It is designed to support the living area for the person receiving the care. Anything outside of laundry or light housekeeping, like dusting and vacuuming, and keeping the area of the care recipient clean, is not a part of respite. The agency has the responsibility tonotify JF&CS Staff if the caregiver/family representative is requesting services that are not meeting the focus of this grant.
11. The Caregiver/family representative must agree to maintain the boundaries of the services and if not, this program is not designed to meet their expectations. As a result, the caregiver/family representative understands that they will be discharged from The ARC JF&CS Respite Voucher Program.
