READ THIS INFORMATION BEFORE COMPLETING THE APPLICATION

• Vouchers are intended for low-income residents or people fixing neighborhood strays. This is a subsidized program, and LSPCA has to pay the difference for each surgery. If you have the means to have your pet fixed, please save a voucher for someone who really needs it. There is not unlimited funding for this program.
  
  
•  For residents of Lancaster County, Pageland, Jefferson, Fort Lawn and Great Falls only. If you reside outside of these areas, you can search for other low-cost spay/neuter options by zip code at this link.
  
  

• You may use this application form for up to four (4) pets for which you are requesting spay/neuter vouchers. If you need a spay/neuter for more than four pets, please complete another application for the remaining ones.
  
  
• Spay or Neuter Service: $30.00 for males / $40.00 for females plus $5 each for any needed vaccines. Your portion will be collected at the vet's office at the time of the procedure. The voucher is valid for at our veterinarian partner, Healthy Pets Spay, Neuter & Wellness Clinic in Rock Hill, SC only. You must be able to provide your own transportation to and from the procedure in Rock Hill. 
  
  
• Note that stray/feral/unfriendly or any difficult-to-handle cats must arrive at appointment in a wire trap, NOT a standard cat carrier for the protection of the staff.
  
  
• If your pet is not up-to-date on vaccinations, they will be provided at a reduced cost prior to surgery: Rabies vaccine: $5 and necessary adult booster vaccines are $5 each. If your pet is current on vaccines, just bring proof with you to the surgery appointment.
  
  
• Additional fees will apply if the pet is in heat, pregnant, or has evidence of fleas. 
  
  
• You will be responsible for paying the veterinary hospital at the time of surgery for the services selected AND any other optional services you add. A $25 non-refundable deposit will be collected at the time of scheduling the appointment. Deposits will be applied to your portion of the fees at the time of surgery.
  
  
• Once your application is processed, a voucher will be sent (usually within 7-10 business days). You can contact Healthy Pets directly to set up your appointment after you have received your voucher(s). 

• Vouchers expire 120 days from the date of issue.

• Note that voucher recipients' names may be shared for billing/reporting purposes.

  
  IMPORTANT Notes: 
  - Additional charges will apply if you select any optional services from Healthy Pets.
  
  - Kittens must weigh at least 2 lbs to undergo surgery, and puppies should be at least 6 months.
  
  - If you do not show up for your scheduled appointment, we will not issue another voucher.
