I acknowledge I am over the age of eighteen (18) and that I have truthfully represented to my tattooer that the obtaining of a tattoo is by my choice alone. I consent to the application of the tattoo and to any actions or conduct of the representatives and employees of the tattoo shop reasonably necessary to perform the tattoo procedure in a professional and safe manner.

 

If I have any condition that might affect the healing of this tattoo, I will advise my tattooer. I am not pregnant or nursing. I am not under the influence of alcohol or drugs. 

I do not have medical or skin conditions that may interfere with said tattoo. If I have any type of infection or rash anywhere on my body, I will advise my tattooer.

I acknowledge that infection is always possible as a result of the obtaining of a tattoo, particularly in the event that I do not take proper care of my tattoo. I have received aftercare instructions and I agree to follow them while my tattoo is healing. I agree that any touch-up work needed, due to my own negligence, will be done at my own expense.

 

Deposit

I understand that all tattoo appointments require a deposit. The deposit covers the consultation and sketch fees.
This deposit is subtracted from the price of the tattoo on my appointment date (final session if it is a multiple session tattoo).  This deposit goes toward the project discussed at the consultation and that no more then two rounds of modifications

will be done.

I understand that if I don't show up for my scheduled appointment or if I don't give the tattooer a 72 hours notice of cancellation, I will lose my deposit and will be required to give a second deposit for a new appointment. 

 

I understand that all deposits are final, non-refundable, and cannot be transferred for use by another person.

 

I understand that on my appointment date, I must be present a few minutes early to discuss any last minute questions I may have and that If I arrive more than 15 minutes late, my appointment may need to be rescheduled so my tattooer will not be running late for the appointments after mine. If my appointment has to be rescheduled due to my late arrival, I will forfeit my deposit and be required to lave another deposit before scheduling a new appointment.

I understand that If the design is unclaimed 6 months after the consultation date, I will forfeit my deposit.

I understand that the tattooer may choose to discontinue our work relationship, if he/she feels that there has been a lack of respect of the shop’s time or a lack of recognition of the value of the rendered services on my part.
