114-2學術活動補助(1)論文發表補助費申請表

一、本次受理申請時間為114年10月1日(星期三)至10月15日(星期三)。

Applying period: 2025 Oct. 1st~Oct. 15th

二、本次受理申請方式：

Application Method:

(一)申請人請「填妥」本申請表、「上傳附件」後，再按「提交」。

Applicant must fill out this form, upload attachments, then submit.

請注意：本表格「無」暫存功能，敬請備齊所有相關檔案後再行填寫作業。

Notice: This form does not hold and save, please prepare all of the necessary files first.

(二)第14項「上傳附件」：請將下列佐證文件彩色掃描並依序合併成PDF檔後上傳。

No.14: Please color scan the documents listed below and combine them into a pdf then upload.

4. 已繳費之收據影本及信用卡繳費金額（俾便確認台幣金額）

(三)提交後，線上申請表會自動將「申請表之PDF檔」寄至申請人之電子信箱。敬請申請人將申請表之PDF檔雙面列印並完成第19項「申請人簽章」及相關主管簽章後，於114年10月17日(星期五)下午5時前送至研究發展處(行政大樓6F)彙整。

After submission, the system will automatically send the PDF version of the form to the applicant's email. The applicant must print both sides of the PDF file and complete No.19(applicant and director signature), then send to the office of R&D before Oct. 17th,2025.

三、紙本申請表送至本處後，如欲更新表單，請重新填寫線上申請表，並於第15項勾選「更新申請表」，無須重新遞送紙本申請表。

After turning in the paper version of the form, any update is needed, please fill out the online form again, and check No.15. There is no need to turn in a paper version.

18. 依據本校學術活動補助要點審查規定之第二點第六款：獲經費補助者若有同時申請或領取其他校外或校內補助，應主動提出，未主動提出者，將由研發處簽請校長核定註銷或酌減已獲補助經費。If the applicant has recieved grants from NSYSU or outside, he/she should report back, if unreported, the office of R&D will deduct the amount of subsidy or cancel the subsidy.*
