    1. I have read and affirm support of the mission of Attorneys for Animals (AFA).
    2. I understand and acknowledge that AFA does not provide malpractice insurance for attorneys who volunteer their services.
    3. I agree to indemnify and hold harmless AFA, its officers, directors and members for any and all claims, demands, actions, liabilities, expenses or losses resulting in whole or in part from my handling of any matter referred to me by AFA or by my failure to comply with the disclosure requirements in this application.
    4. I agree to notify AFA immediately of any pending disciplinary proceedings and of any past disciplinary proceedings closed in a manner adverse to me (including matters closed by private reproval).
    5. I agree to notify AFA of any changes to affirmations or statements in the above application within a reasonable time.
    6. I have read and understand the Disclaimer to the Animal Friendly Attorneys™ program, including but not limited to the fact that the selection of attorneys, and their continuing participation in the program, is within the sole discretion of AFA’s board of directors.
    7. I agree to update my answers to this application and to provide additional information that from time to time may be requested by AFA.
    8. I agree to become and maintain membership in AFA as an Attorney member.
    9. If selected, I agree to use the Animal Friendly Attorneys logo and name only in accordance with AFA’s brand usage guidelines, which may be updated from time to time, and at AFA’s sole discretion.
