一、本次受理申請時間為115年5月1日(星期五)至5月15日(星期五)。

二、本次受理申請方式：

(一)申請人請「填妥」本申請表、「上傳附件」後，再按「提交」。
Applicant must fill out this form, upload attachments, then submit.
請注意：本表格「無」暫存功能，敬請備齊所有相關檔案後再行填寫作業。

(二)第13項「上傳附件」：請將下列佐證文件彩色掃描並依序合併成PDF檔後上傳。
No.13: Please color scan the documents listed below and combine them into a pdf then upload.
1. 投稿證明(且須能證明以中山大學名義發表)

2. 2024年期刊排名證明文件

3. 已繳費之收據影本及信用卡繳費金額（俾便確認台幣金額）

(三)提交後，線上申請表會自動將「申請表之PDF檔」顯示於【感謝畫面】或【寄至申請人之電子信箱】。敬請申請人將申請表之PDF檔雙面列印並完成第18項「申請人簽章」及「系所主管簽章」後，於115年5月20日(星期三)下午5時前送至研究發展處(行政大樓6F)彙整。
After submission, the online form will automatically in thank you send a pdf version of the file to the applicant's email. The applicant must print the pdf file and complete No.18 (applicant signature and department director signature), then bring the file to the office of R&D before Oct. 17th,2025 5p.m.
三、紙本申請表送至本處後，如欲更新表單，請重新填寫線上申請表，並於第14項勾選「更新申請表」，無須重新遞送紙本申請表。
If an update is needed after turning in the paper copy, please fill out the online form and check No.14 "Update form". There is no need to turn in another paper copy.
