• Adult ADHD Symptom Checklist

  • The questions on the next page are from the Adult ADHD Self-Report Scale (ASRS-v1.1), and have been formatted into this survey tool to allow you to assess your symptoms. These questions are designed to stimulate dialogue between you and your doctor and to help confirm if you may be suffering from the symptoms of ADHD.

    Complete the Symptom Checklist by choosing a response that most closely represents the frequency of occurrence of each of the symptoms. Once you submit you will see the results, which will tell you whether or not you have symptoms consistent with ADHD. This is not meant to diagnose any condition. Instead it is meant to provide you with information to use when speaking with a licensed professional who is qualified to evaluate and diagnose ADHD. A proper diagnosis requires a detailed evaluation by a qualified provider.Β
