    Thank you again for your support of our community camp. Parents who elected a guaranteed spot(s) last May now have one week (starting January 11, 2021) to either confirm or change (if they want) their previously chosen sessions and submit payment (only for additional sessions), before registration opens for the other returning families.

    Your spots are guaranteed in 2021 only for the session(s) selected in 2020. You do not need to resubmit payment for previously chosen sessions (same sessions as 2020) for returning campers.

    Any changes to your 2020 sessions (new sessions or additional campers) will require a fee adjustment, according to the process outlined in this form. Your spots, like always, are contingent on space being available. Please wait for a confirmation email from the Registrar about these changes.

    Returning families can now complete this application. You have only two weeks, space permitting, to complete your application before registration is opened to the public on February 1, 2021.   • RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS & INDEMNITY AGREEMENT

    I recognize that my participation in the program/activity for which I have registered may include a risk to my health or a risk of injury. I hereby willingly assume such health risk or risk of injury for myself or for the above-named person(s) for whom I am in law responsible, and I assume full responsibility before, during and after my/their participation in the program/activity. In consideration of the acceptance of my application and the permission to participate in the program/activity, I, for myself, my heirs, executors, administrators, successors and assigns HEREBY RELEASE, WAIVE, AND FOREVER DISCHARGE the City of Toronto, all other organizations, associations and companies associated with any of the programs offered by the City of Toronto, and all their respective employees, agents, contractors, consultants, representatives, elected and appointed officials, successors and assigns (all of whom are called the "City Indemnities") OF AND FROM ALL claims, demands, losses, damages, costs, actions and other proceedings whatsoever, whether in law, statute or equity, in respect of death, injury, loss or damage to me or my property, howsoever caused, except to the extent caused by or attributable to the negligent or intentional acts of the City Indemnities, arising or to arise by reason of my participation in the program or any of its associated activities. By using automated touch-tone registration, telephone operator- assisted registration or Toronto FUN Online registration, I acknowledge that I have read, understood and agree with the risk waiver.

    The personal information collected in accordance with this form is collected under the authority of the City of Toronto Act, 2006, ss. 8, 74 &136, and the City of Toronto Municipal Code Chapter 441, Fees and Charges and Chapter 608, Park. The information is used to process your application for program participation; the registration of individuals in programs; payment of fees; collection of outstanding fee amounts; aggregate statistical reporting, contacting clients regarding upcoming programs, and, additional mailings, including newsletters/surveys and email notifications and receipt transactions.   • If {campersName} was enrolled at Earl Bales in 2020, or if {campersName} is a new sibling from your returning family, indicate your desired addition or change for 2021. The appropriate fee adjustment will be calculated below.

    These spots, like always, are contingent on space being available.

  • Extended Hours (DON'T DELAY - SPACE IS LIMITED)

  • Yes! I would like to take advantage of Extended Hours, which allows me to drop my child(ren) off at 8:00 a.m. and to pick my child(ren) up by 5:30 p.m. I understand that Extended Hours are offered on a full week basis only, but I can arrange with the Camp Director for only partial use (for example, just mornings, just afternoons, or just a few days of the week, etc.); however, the $65 per week per child fee still applies. Finally, I understand that I must pick up my child(ren) by 5:30 p.m.; otherwise the camp incurs additional staffing costs. My share of those additional costs after 5:30 p.m. would be $1.00 per minute, per child for every minute after 5:30 p.m. My signature below indicates agreement to these terms.

    NOTE that on the last day of camp (August 27) there will be no extended hours at camp. All children MUST be picked up by 3:30 p.m.

    I agree that the emergency Medical Treatment permission and other Terms and Conditions that I acknowledged via my signature on the application form also apply during extended hours.

  • Payment Process

  • Payment for 2021 camp fees will be accepted through Interac e-transfer only . Divide {campersName} 's total fees of $ {total2021} .00 into two payments, as follows.

  • 1. First Payment

    Submit a first payment for {campersName} in the following amount as soon as you complete this form. Only applications with a completed first payment will be considered for acceptance to the camp. Please note: this per-camper first payment includes a $150 non-refundable deposit per camper.

    For all e-transfer payments:

    • Use the email address ebfees@gmail.com as the recipient.
    • Use your camper's postal code (no spaces) as the security answer .
    • Include your camper's full name ( {campersName} ) in the message field. Payments without a camper's name attached will not be accepted. Only one camper per transaction, please.
  • 2. Final Payment

    Submit the remaining balance only after receiving confirmation of a spot for {campersName} from the Registrar. Full payment must be received by May 17, 2021.

    For all e-transfer payments:

    • Use the email address ebfees@gmail.com as the recipient.
    • Use your camper's postal code (no spaces) as the security answer .
    • Include your camper's full name ( {campersName} ) in the message field. Payments without a camper's name attached will not be accepted. Only one camper per transaction, please.


    Please note: maximum transactions and daily transfer limits vary, based on the agreement you have with your own bank. If you can not send the final payment in one transaction due to your daily transfer limit, multiple transactions will be accepted but only until the deadline. Plan ahead, do not delay!

    For example:
    Camper A enrols for the entire summer and his family's daily transfer limit is $1000. After submitting the first payment, the remaining balance is $2165, which is over $1000. So his family will have to send multiple payments:

    1. First Payment: $400 (already received)
    2. Next Payment: $1000 on May 15
    3. Next Payment: $1000 on May 16
    4. Next Payment: $165 on May 17
  • A receipt will be mailed after the camp session is completed. Please keep your receipt for income tax purposes. A duplicate will NOT be issued if lost or misplaced.

    1. Applications without proper payment will not be processed.

    2. If you require special consideration, please call 905-707-5821.

    3. Full fees (less $150 non-refundable deposit PER CAMPER) will be refunded up to and including May 1. NO FEES WILL BE REFUNDED AFTER MAY 1.

    4. As our fees are for specific sessions, there will be no refunds for days missed. A doctor’s certificate will be required for an absence due to illness for two consecutive weeks or more in order that we may consider refunding 50% of the remaining fees.

    5. Fees for days missed are not transferable to other sessions or to other families.

    6. Earl Bales Community Day Camp reserves the right to refuse enrolment or dismiss a camper if it is in the best interest of the camper or the camp.

  • Release

  • I hereby release the City of Toronto and the Earl Bales Community Day Camp staff and Parent Committee from all claims for damages arising from any accidents or injury which are caused during participation of the camper named herein during any program, in any facility, or at any location where a program is being held.

    I hereby give permission for said camper to participate in all camp activities and trips (fully supervised) both on and off camp property. I hereby give camp Directors full authority to act on my behalf in case of an emergency.

    I have read and I understand the above, and the Conditions of Enrolment and hereby agree to all terms and conditions in this application and the Conditions of Enrolment. I further certify that all information is true and correct.
