2 entries per person. No 2D art larger than 12x12. No 3D artwork over 12x12 and weighing more than 5lbs. All fields must be completed. (Exception: if you are not entering (2) pieces, you do not need to enter information for the second piece. PLEASE DO NOT ENTER TWICE, it will be deleted. All entries must be on one form, per artist.
