- Name of the applicant & confirm he/she is an active member in your SSA Chapter or Organization

- Provide the number of your Organization’s members & the current number of students being trained
- Is the Chapter or Organization interested in growing its training efforts ?

- What is the Chapter or Organization’s need for instructors?
